Obviously he is suggesting they are not carrying out his instructions.
In my experience, ignoring a gaffer's advice may come about for a variety of reasons, among them:
The instructions are unclear and therefore not successfully implemented
The instructions have never actually been issued
The instructions are so daft that the consensus is they are best ignored
The instructions are fine but the person issuing them is a plank, so they are routinely ignored in any case
There are no meaningful repercussions should said instructions be ignored so why take the trouble
Obviously this list is far from exhaustive. I just wonder whether any of the above could be relevant to our current predicament given that the same problems seemingly crop up like a recurring nightmare...

I was picking him out of the bunch as an example of a player that gets played out of position....

How about some players are so flipping thick, they're incapable? I just wonder if someone did come in, how many players would fit into the category, don't bother banking him off again, you're wasting your time, get rid.

Maybe LR is over complicationg the intructions to the players, and they dont bother as such.
There does not seem to an organised set pattern, although, how hard can it be for them to man mark `player x` at a free kick, and put a guy on each post?

The players can't get the basics right, never mind the more taxing stuff.
