Honestly don't think I've ever been banned from anywhere. Thrown out plenty but never banned.

EDIT: I was also banned from a bowling league in Belgium, me and my mate Dav turned up one night to bowl in the Brit league wearing snorkels, masks and fins. We were ejected forcefully and asked never to go back and recind our registration.
Must be something about bowling alleys

Oh and I was banned from the Rutland after getting leathered and causing a scene, I then broke into work and got sacked shortly after. Good times.
