I suppose the official mechanism is "knock on the ref's door with a cup of tea..." You'd like to think that the ref got an email from Ref plc telling him to keep an eye on it after the first leg.

Most refs, but not all, will have a pre match brief with the managerial team, usually when team sheets are handed in. This could be a one way or two way brief depending on the ref.

It's a different kettle of fish though in fairness, yellows for timewasting like the lad on 36 minutes are pretty common place even early in the game, and whilst it cuts it down for the remainder of the match, there are another 9 players who can take advantage and take the hit with a yellow for delaying taking a throw, or taking a kick (GK included).
When the GK goes down with a supposed injury, the Referee's hand are tied as he cannot fully prove it is a delay tactic. The only way forward is after a 3rd occasion of going down, the Referee suggest that he is not happy to let the Keeper continue to safe guard him, a bit like the concussion subs, it could even be an extra stoppage and extra sub. Just simply on the 3rd occasion down, let him and the physio staff know that if he goes down again, he will need to be removed and replaced by the sub keeper
